To secure your place, you will need to provide the following documents.

Kinder to JHS

  • Photocopy of Birth Certificate (PSA Copy)
  • Photocopy of Baptismal Certificate
  • Original copy of Form 138 (Report Card)
  • Certificate of Good Moral
  • Passport Size ID picture (2 pcs)
  • Medical Certificate (HTU Clinic)
  • Alien Certiciate of Registration (ACR) for foreign students only

Senior High School

  • Recent 2×2 ID Photo (2 copies)
  • Certified Birth Certificate (PSA Copy)
  • Original copy of Grade 10 Report Card
  • Certificate of Good Moral Character
  • Photocopy of Baptismal Certificate
  • NCAE Result
  • Entrance Examination Result

College (Freshmen / Transferees)

  • Photocopy of Birth Certificate (PSA Copy)
  • Photocopy of Baptismal Certificate
  • Original copy of Form 138 (Report Card)
  • Certificate of Good Moral Character
  • Passport Size ID picture (2 pieces)
  • Photocopy of Transcript of Records or Class Card (for Transferees)
  • Transfer Credentails (for Transferees)
  • Medical Certificate (HTU Clinic)
  • X-Ray and Hepatitis tests (for BSHRM & BSN)
  • RLE (Related Learning Experience) Records (for BSN Transferees)
  • Alien Certificate of Registration (ACR) for foreign students only

Graduate School

  • Recent identical 2 copies of passport size ID photo
  • Photocopy of PSA certified marriage contract (for female married status)
  • Transcript of records and transfer credentials
  • Photocopy of PSA certified birth certificate (for female single / male married)
